Boone, Campbell, Grant and Kenton counties have been added to the  Cincinnati-Dayton region  by the Centers for Medicare and Medicaid Services (CMS) as one of seven geographic markets to carry out the CPC initiative.

Up to 75 practices will be selected within the region to participate—your practice could be one of them! Physicians in these practices will receive a monthly care management fee averaging $20 per month for each Medicare patient to support enhanced primary care services. This is in addition to traditional fee-for-service reimbursement. Ohio Medicaid and nine other private payers will also be participating in this blended payment model.

Application Process
  1. Complete the application pre-screening tool. This non-binding questionnaire will help determine if your practice is eligible to apply. (Note: the pre-screening tool is NOT the actual application.)
  2. If you are found to be eligible to participate, you will receive an email from CMS with a link to the application and other important information.
  3. Submit your application by 5:00 p.m. EST on Friday, July 20.
