The CPC initiative will offer blended payment (care coordination fee plus fee-for-service) from public and private payers to these primary care practices selected to participate in the CPC initiative. In addition to Medicare, participating payers include Aetna, Amerigroup ( Ohio only), Anthem Blue Cross Blue Shield of Ohio, Humana, UnitedHealthcare, CareSource ( Ohio only), HealthSpan, Medical Mutual, Centene Corporation ( Ohio only), and Ohio Medicaid (Aged, Blind, and Disabled).

The Kentucky Academy of Family Physicians worked diligently alongside the American Academy of Family Physicians, the Ohio Academy of Family Physicians, the CMS Innovation Center , the Health Collaborative ( Cincinnati ), and other stakeholders to recruit practices in the market to apply to participate. Members in the market received frequent messages from the KAFP about this opportunity.
