The Bluegrass Community Health Center in Lexington, Ky received Level 2 Patient-Centered Medical Home recognition from the National Committee for Quality Assurance (NCQA) this month. BCHC is the first Kentucky based CHC to achieve such recognition. Founded in 2001, BCHC began the process of recieving recognition in May 2011. At that time, the Executive Director, Susan Fister, PhD, a nursing professor at Eastern Kentucky University, Lauren Bishop, RN, MPH, the Quality Director and Steve Wrightson, the Medical Director attended a conference on PCMH sponsored by the Ky Promary Care Association (KPCA). The medical providers who helped with this transformation are Tom O’Neal, Maria Castro, Lydia Mustafic and SreeVali Chamarthi, all Family Physicians, as well as Robin Smith, APRN. Invaluable support came from our mental health providers, Liz Epperson, Jason Reich and Julie Clarke, and all our nursing, clerical, and administrative staff.
