The Kentucky Academy of Family Physicians is accepting nominations for the 2015 Exemplary Teaching Awards. These awards were developed by the Board of Directors of the Kentucky Academy of Family Physicians to acknowledge Academy members who deserve recognition for excellence in teaching, as well as individuals who have implemented outstanding educational programs and/or developed innovative teaching models. One state award will be presented in two separate categories; full-time or part-time faculty and volunteer faculty.

Members will have the opportunity to nominate one family physician in each of the two categories. Only KAFP chapter members may submit nominations.  Members are encouraged to solicit nominations from family medicine department chairs, residency program directors, residents and students. The awards presentation will take place at the KAFP 63nd Annual Scientific Assembly at the Campbell House in Lexington, KY on Nov 12-14, 2015.
