Which Forms Do I Use for the DOT Driver Exams?
Until December 22, 2015, Certified Medical Examiners are required to continue to use the Medical Examination Report Form and Medical Examiner’s Certificate found on the Federal Motor Carrier Safety Administration website found below:
After that time, Certified Medical Examiners will be required to use the new Medical Examination Report (MER) Form, MCSA-5875 and Medical Examiner’s Certificate (MEC) Form, MCSA-5876. FMCSA will be responsible to update the CMV Driver Medical Examination Results Form, MCSA-5850 used by Certified Medical Examiners to report CMV driver examinations through their National Registry account.
When Must I Report “No Exams Performed”?
It is now a regulatory requirement for Certified Medical Examiners to report to FMCSA, by close of business on the last day of the month, whenever the ME does not complete any examinations during that month.
